Leeds and Grenville partners have secured $3.6 million in federal funding to expand community-led initiatives addressing substance use and addiction.
The funding comes from Health Canada’s Substance Use and Addictions Program and will support the Summit Integrated Care initiative. Summit provides a single point of access to health care and referral services through a multidisciplinary team that includes community paramedics, health system navigators, and a nurse practitioner.
Operating under a harm-reduction model, Summit offers flexible service hours, including evenings and weekends, to help participants access education and resources that support safety, stabilization, and recovery.
Day-to-day operations are led by the Leeds Grenville Community Paramedic Program’s Summit Integrated Outreach Team, with Southeast Public Health overseeing financial management and data reporting through March 2028.
Since launching in April 2025, the program has enrolled 239 participants, with numbers continuing to grow through referrals and proactive outreach. The initiative works closely with community partners to deliver care where it is needed most.
